Case Manager

Job Overview: Florida Outreach Center for the Blind, Inc. (FOCB) seeks to hire a Case Manager to assist blind and visually impaired adults with reaching their highest potential. The Case Manager will work under the supervision of the Executive Director and must be committed to the organization’s mission: “To develop and administer programs that will integrate blind and visually impaired persons into the social, economic, and spiritual lives of their community.”

Case Manager Job Responsibilities:

  • Accomplishes clients’ care by assessing needs; developing, monitoring, and evaluating wellness plans and progress; facilitating interdisciplinary approaches.
  • Determines clients’ requirements by completing intake interviews and assessment of skills; develops a comprehensive training and wellness plan.
  • Assists client with establishing training programs by setting schedules; coordinating services being provided; arranging linkages with community partners, including transportation.
  • Monitors cases by verifying clients’ attendance; observing and evaluating students’ progress and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
  • Maintains clients’ records by reviewing case notes, logging events and progress. Responsible for completing and submitting monthly reports to the Division of Blind Services (DBS).

Work Hours/Compensation: 32 Hours per week; $21.00/hr. ($35,000 Annual)

Education, Experience, and Licensing Requirements:

  • Associate’s degree in social work or related field
  • Customer service experience, preferably in human services
  • Professional or personal experience supporting individuals with disabilities, visually impaired experience preferred
  • Must have a valid in-state driver’s license, provide current registration and insurance.
  • Must be over 21 years old
